Evaluates patient’s response to treatments/medications. * Actively participates in Quality management program. * Completes accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion. Qualifications * Graduate of an accredited nursing program. * Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in state of practice. * Master’s Degree in ministry, counseling, or social work preferred. * Clinical Pastoral Education (CPE) training preferred. * Two (2) years or equivalent full time experience in bereavement counseling within the last three (3) years. * Valid state driver’s license and reliable automobile, current automobile insurance, and be willing to operate personal car necessitated by nature of job. * Experience as an LPN/LVN commensurate with one of the following preferred: * o One year experience in an acute care setting within the last 24 months; * o Six months home care experience within the last 12 months preferred. * Valid driver’s license and reliable transportation * Proof of automobile insurance (MVR checked) * Proof of current CPR certification * Able to pass criminal background check and national sex offender clearance
