Description
Join our dedicated team at Hospice of the Piedmont as a full-time LPN who, under the supervision of a registered nurse, delivers quality patient care and comfort measures, emphasizing physical, spiritual, and emotional needs.
Work hours are Monday through Friday 8:30am - 5:00pm.
Responsibilities/General Duties
Deliver nursing services per Hospice policies
Document care using EMR
Assist physicians/RNs with procedures
Administer medications/treatments as prescribed
Observe, record, and report patient conditions to RN
Follow Hospice Plan of Care for patient/family services
Report changes in patient needs to RN case manager/team leader
Explain Hospice services to patients and families
Communicate with Interdisciplinary Team for coordinated care
Participate in QA programs and required meetings
Engage in professional development and maintain affiliations
Maintain patient confidentiality
Qualifications
Licensed as a Licensed Practical Nurse in the Commonwealth of Virginia
Two years’ relevant nursing experience preferred
Ability to manage all aspects of terminal care, health team service coordination, and documentation
Intermediate proficiency in word processing software
Driver’s license, personal automobile, and current automobile liability insurance

Drive long distances to homes and facilities
Must be prepared to work in home environments that have difficult working conditions including, but not limited to cramped space, pets, dirt, allergens smoke, limited seating
The job requires regular use of hands for handling objects, tools, or controls, and involves sitting, standing, bending, reaching, lifting, walking, and climbing steps. Vision requirements include close, distance, color, peripheral, depth vision, and the ability to adjust focus
It may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care
Communication by phone, writing, speaking; Regular use of computer screen and keyboarding are required
Able to push, pull, or lift 50 pounds, push up to 200 pounds in a wheelchair
